In order to ensure that your visit is as swift as possible, please feel free to print and complete any of the following forms applicable to your visit. Arriving with all necessary paperwork greatly assists our staff in creating and updating your file while limiting your overall wait time.

Office Policy

Office Policy on Missed Appointments:

There will be a $75.00 charge for appointments that are not cancelled two full business day in advance.  Patients may be required to pay this fee before future appointments are scheduled.

Any appointments made between the hours of 2:00 p.m. and 6:00 p.m. are considered “preferred appointments time slots” and must be cancelled two full business days in advance. If an appointment is cancelled in less than that time the patient’s next appointment must be between the hours of 9:00 a.m. and 1:00 p.m.

Repeated failures to keep scheduled appointments (for example, no more than 3 in a 12 month period) are considered grounds for immediate dismissal from the practice.​

Office Policy on Medication Refills:

It is the patient’s responsibility to ensure that they have sufficient supplies of medications to last until their scheduled appointment.  Dr. Krulee reserves the right to deny refills of medication for patients if they are delinquent in keeping their appointments and have not been seen at the office in person.

HIPAA Policy

At the Landmark Center for Behavioral Health, we strongly adhere to the privacy rules of HIPAA (Health Insurance Privacy And Portability Act).  The HIPAA Privacy Rule provides federal protections for personal health information and gives patients rights with respect to that information. At the same time, the Privacy Rule allows for the disclosure of personal health information when it is necessary for patient care or other important purposes.  Before your first appointment at the Landmark Center for Behavioral Health you will be required to sign a form confirming your understanding of our privacy practices and adherence to HIPAA Policy.
